Digitizing patterns - what tools?

When you digitize a pattern, what tools and software do you use to convert your paper pattern to a digital pattern?

No I mean a printed knitting pattern.

Arent they mostly just text patterns? Could you not just use scanning software and OCR them? MS OneNote might do this. It can OCR some images for you.
Though, I suppose the knitting abbreviations might make it harder.
