Hmm my screen doesn't have those options.
The image doesn't show up when I save the draft either.
Take a look at this screenshot of a thread I'm moderating now:
After clicking on attach (to upload the file), you'll click on select attach and choose the file on your computer. Then, after the upload is complete, you'll see a place to put the price like this picture above.
Trying to edit what I uploaded to get it right but...
I definitely do not have the line that starts with the pdf symbol file name description read permissions gold coin line.

This part works:

This part is missing my pattern says 100% but is not clickable no boxes.
Then, after the upload is complete, you'll see a place to put the price like this picture above.
Everything else in your image is the same.
My image is missing also.
I think others might be having the same issue since there are other patterns awaiting approval with a missing image just like mine. At least that is how it looks on my mac. Some patterns have images some don't.
I've been reporting to the administrator all users who have such problem. She says she's talked to the technical team about it but it may take some time to be solved. Let's be patient, please.
