I start at the top left hand corner. I grid my fabric but I will try starting on the middle so I wont have to grid anymore. :)

It depends on the pattern, if an odd shape in the middle, for something very rectilinear top left so I can get more out of the fabric

I usually start in the middle as well, and try to do all of one color at once.

I dont like gridding the fabric because I just want to jump right in and stitch. I only grid the middles. So the I find the middle running north to south and the middle running east to west then start they intersect I start stitching.
