since I usually stitch giant pieces I start at the top left.  This is usually page one.

Always in the very center.... one exception I made was to start with a border but don't recommend it. One wrong stitch count and you have to start all over...

I grid too ! I like to start with the largest patch of color or the most prominent part of the design and work my way around it.
